We were having an issue with the forum emailer for the past month or so, not sending emails randomly. Possibly being due to the fact of maxed out on emails per day Gmail's SMTP. To test that i reset all subscriptions to no notifications to weed out old users hogging the load.
Sorry for any inconvenience. You can set subscriptions to any thread you wish manually that were lost before.
